FOR YOUNG PEOPLE COMING TO TEEN CENTRAL OR CLUB FOR THE FIRST TIME

If you would like to sign your child up for membership at Teen Central or Club, please fill in both the forms below. Applications cannot be processed until both forms are submitted. The information should be submitted by a parent or guardian and will be sent securely to our office. We will contact you to let you know if your application is successful.

St Mike's Youth Clubs Sign up Form

We will use this information to contact you if there is an emergency regarding your child at Youth Club, or if the behaviour of your child is not acceptable. We will also use this information to contact you and your child with information of relevant activities within the youth ministry. We will store the data electronically and securely until the September after your child turns 18. If you do not consent with any of these points, please state so on the form.
