About the SMY Parents Questionnaire

Dear Parents/Guardians

We want to gather some information about how we can better support you and your young person/people as we all transition back to a new normal with less and less restrictions.

We want to assess how people are doing so we can better serve you guys and help us to know what is going to be the most useful thing for us to spend our time on over the next few weeks and months.

The information provided will be stored securely and you do not have say who you are. (However it might be useful for us to know who you are if you answer question 2 with the last option.) We will only use this information internally within the St Mikes Youth Team and St Michael's Staff team to help us make better decisions moving forward.
